Coinsurance per day:
Days 61-90:	$256 (up from $248)
Days 91-150:	$512 (up from $496)
60 lifetime reserve days Medicare pays 100%
SNF Coinsurance:	$128.00 per day for days 21-100

Medicare Part B

Premium per month:	$96.40 (up from $93.50)
Deductible per year:	$135 (up from $131)
