  • Advance Health Care Directives (AHCD) are meant to give adults control over their health care if there ever comes a time when they can no longer make choices on their own.
  • Because the form is supposed to reflect each individual’s preferences and wishes, there can be and really should be variation among the forms. ¬†Several organizations, law firms, and individuals offer AHCDs, each having slightly different features.
  • CANHR’s form includes significant and unique limitations on a health care agent’s authority in Section 1.25 that should be considered carefully.
  • CANHR recommends that anyone filling out an AHCD review the choices with their health care provider and an attorney before executing the form.
