CANHR posts Advance Health Care Directive for Consumers

Every adult should have an advance health care directive (AHCD) to state their health care preferences and to name a substitute health care decision-maker in case there is ever an accident or condition that renders the adult unable to make their own decisions.  California has long-suffered from a lack of free, widely available, high-quality AHCDs.  To remedy this deficit, CANHR has created its own AHCD form, free for download.  The form has innovative features specific to long-term care concerns as well as carefully constructed but simple expressions of end-of-life wishes.  Consumers can also download a detailed set of instructions for guidance in completing the form.
